Effective digital solutions are an integral part of personalisation, giving people the ability to access services and monitor their own wellbeing. For commissioners, the data provided by the workforce, gives them and health and care practitioners vital insights into the outcomes for local populations and the health inequalities that exist.
The data tells us where the most need is and what it looks like; and whether we have the capacity to meet the needs of local people.
Digital solutions should enable choice and control that in turn leads to improved wellbeing and which reduces reliance on crisis services. They can also support greater integration across services and integrated care, and support planning across health and social care.
In north east London our work on developing digital solutions aims to:
- Enable safe and effective record capture and keeping
- Create data foundations for reporting, communication and quality improvement
- Provide the service user/ carer with the means to self manage and personalise care
Access the latest data on north east London social prescribing work
Access to the dashboard is being provisioned over the coming weeks to PCN Clinical Directors and PCN managers. By seeing this rich data it will help them meet the updated PCN DES requirement to undertake proactive case finding for social prescribing, and to easily submit their social prescribing activity reports.

Proving social prescribing works
The NEL Social Prescribing Evaluation Group is led by Dr Victoria Tzortziou-Brown whose focus is research and evaluation work. The evaluation group is helping to ensure we are delivering high-quality evaluation of our work alongside creating a north east London-wide community of practice.

Digital support for our social prescribing teams
Over the past year, the north east London Evaluation group has worked towards implementing a minimum dataset for Social Prescribing. It includes: Patient/Resident demographic, Presenting issue, Intervention, Patient Recorded Outcome Measure data.
The dataset has been rolled out via templates in EMIS and SystmOne. During 2022 we're continuing work to embed the template across NEL and we welcome your feedback.
